How do I make it to where like this please, thank you:

EMail Subject: Click Down and choose for example:
I Love McDonalds
Other/Others/Mix/Mixes
I Love Brocolli

Message Box: Unlimited Characters can be put in this message box.

Send: It will send to my EMail Address once Send is clicked on.
